Applying Impactful Analytics to the World of Population Health

Population health management. We all have our own definitions of what this is and how to get our arms around it. But in this world of new technology in healthcare with more data than we ever had before, how do we even approach the topic? At Lehigh Valley health network in Allentown, PA, we have employed data analytics to address population health from many aspects – chronic disease management, payer populations, in-house and long term pathway management. We have deployed analytics across the continuum of care to ensure impactful results. By weaving analytics into the core of our EHR and clinician workflow, we can guarantee success both now and in the future
